Physician and Hospital Finder: Locate in-network National POS Open Access providers available in your area and throughout the U.S., including Mental Health and Substance Abuse providers. The network does not require referrals when you access specialized care.
If you cannot locate your physician using Humana's Physician Finder Plus, please call your physician's office to verify if he/she is contracted with Humana ChoiceCare. If your provider is not contracted, you may nominate your physician for our network. There are two ways to submit nominations: Online or go to the Forms page and complete the Provider Nomination Form as requested for the nomination process. You will also find a Nomination Notice Letter to take to the hospital or physician to notify them of the nomination.
Enrollment Hotline: Call us toll free at 888-393-6765 for answers to your questions about the Humana options and benefits offered during annual enrollment. This line is also available to new hires during their enrollment period.
Customer Service: Once you are enrolled, you will find convenient access to our Customer Service Representatives from 8 a.m. to 6 p.m. ET, Monday through Friday, and our automated information line is available 24 hours a day, 7 days a week.
